Initial Evaluation - $300

Standard Follow-up Appointment - $180

Extended Follow-up Appointments - $280

*Longer appointment times are available upon request

*A deposit of $150 is required in order to book on the schedule as a new patient. This deposit will be applied towards your total initial evaluation fee

*As an out-of-network provider, I do not accept insurance. Acceptable payment forms include cash, credit card, check, HSA card. You may use your insurance at the pharmacy for medications/prescriptions and labs.
